Customer Installation Checklist Pre-Installation (to be completed with Customer) _______ Performed a Pre-Installation walkthrough with customer. _______ Confirmed antenna mounting location is acceptable with the customer. _______ Reviewed configuration of all TV outlets prior to installation. _______ Reviewed functionality of all hardware and possible upgrade options prior to installation. (i.e. UHF). _______ Reviewed with customer, the orientation materials provided, including the “Welcome to DISH Network” booklet, Programming Brochure, Remote Control Guide, Channel Lineup Card, and the Installation Comment Card. Introduction to DISH Network (to be completed with Customer) _______ Reviewed setup/programming of the universal remote to operate other equipment. (i.e. VCR) _______ Introduction to Channel 251 and User and Installation Guides. _______ Demonstration of remote control functions and introduction to the Remote Control Help Channel 101. _______ Demonstration of how to access the Electronic Programming Guide and use the Favorites List and timing functions. _______ Reviewed ordering Impulse and Non-Impulse Pay-Per-View events and movies via the phone line. Explained all advantages of permanent phone line connection phone line was connected using most efficient method. _______ Reviewed selection of alternative language simulcasts. _______ Reviewed all programming options available within current promotional offering. _______ Reviewed local channel programming availability and explained 2-dish solution to receive all local channels. _______ Reviewed local channel(s) location on the guide, if locals have been purchased, and introduced to Locals Information Channel. _______ Reviewed options available for using an off-air antenna, if local channels are not included in the current local programming package. _______ All receivers are connected to a phone line in order to best utilize hardware functionality. (Provide additional comment below if phone line could not be connected) _______ Customer is aware of the importance of Surge Protection. Post-Installation (to be completed by Technician) _______ All necessary paperwork has been reviewed, signed, and collected. (DNSC Service Agreement; DHP Customer Agreement; DHP Certificate, etc.) _______ If customer is participating in the Digital Home Plan as indicated on the work order, reviewed with the customer the benefits of the In Home Service Plan and first month of subscription programming ordered, is included in the activation fee. _______ Reviewed with customer local network programming options and 2-dish solution to receive all local networks. Provided customer information on off-air antenna availability and setup. _______ Reviewed with customer billing information. ( Payment options, First bill is for 2 months) _______ Ensure that the customer is happy with the installation, programming, and hardware, etc. _______ The premises were left clean and neat.
